<p>A 59 year old woman has been killed in a three vehicle collision in East Sussex.</p>
<p>The collision involved a van and two cars and took place at around 08:00 BST on the A22 at Uckfield.</p>
<p>The woman was a passenger in a Ford Mondeo Estate.  She died on the spot after the vehicle she was travelling in crashed with a Ford Transit van.</p>
<p>A driver of the Mondeo, a 64 year old male, sustained serious injuries as a result of the collision.  He has been admitted in the Royal Sussex County Hospital.</p>
<p>According to the police, the collision also involved a silver Mercedes.</p>
<p>The driver and passengers of the other vehicles sustained minor injuries.</p>
<p>The Police have appealed for witnesses to come forward.</p>

<p>The collision also left a mother and her toddler injured.</p>
<p>The collision took place in West Sussex and involved a Ford Fiesta and a Mercedes C180 which was being driven by a local woman, 37 years of age.</p>
<p>According to the police, the driver of the Ford Fiesta had died on the spot.</p>
<p>The female driver of the Mercedes and her toddler sustained minor injuries and were taken to hospital.</p>

<p>A demolition company in Birmingham has been prosecuted by the HSE after a worker sustained serious injuries as a result of becoming trapped in a crushing machine which was unguarded.</p>
<p>In the incident which took place on 8 November 2012, a 33 year old plant operator working for Coleman and Company Ltd was injured after his right arm became trapped between the conveyor belt and the rollers.  The incident took place in Cornetts End Lane, at the company’s site.</p>
<p>The worker suffered bruising, wrist injuries and fractures to his arm.  He had to undergo several skin grafts and has not been able to return to work.</p>
<p>On 30 May 2013, Solihull Magistrates’ Court heard that if his colleagues had not acted promptly, he would have lost his life.</p>
<p>According to an investigation carried out by the HSE, it has been found that there were two breaches of health and safety legislation.</p>
<p>It was found that the company had failed to notice that the fixed guards on the machine were lost or had been removed. The company had also failed to replace them to prevent accidents and injuries from occurring.</p>
<p>Coleman and Company Ltd was fined £3,500 in addition to £1,114 in costs.</p>

<p>A lorry driver has been killed following a crash which involved a van, a car and two lorries on the M6.</p>
<p>The collision happened on the southbound carriageway at around 11:00 BST.</p>
<p>The driver of the lorry had died on the spot.  A lady passenger in the car sustained injuries and has been admitted in University Hospital Coventry.  The woman is believed to be in her 40s and sustained neck injuries.</p>
<p>The southbound carriageway was closed for several hours following the accident.</p>
<p>The driver of the other lorry also sustained injuries.  He was treated at the scene by the emergency services.</p>
<p>The van driver escaped unharmed.</p>

<p>The crash which happened on the A689 between Sedgefield and Wynyard at around 11:40 BST on Saturday involved two cars.</p>
<p>25 year old Andrew Dunn, the driver of the green Ford Fiesta died on the spot.  Two occupants of the blue Peugeot sustained bruises and cuts.</p>
<p>The road was closed for almost 5 hours to allow initial investigation to be carried out.</p>
<p>Cleveland Police have requested witnesses to come forward.</p>

<p>The incident took place on Saturday at around 04:00 BST at Commonhead.</p>
<p>The passenger, from Swindon, was in his 20s and was declared dead on the spot.</p>
<p>The incident involved a Ford Fiesta and a lorry and it occurred when the driver of the car hit the rear end of the lorry.</p>
<p>The driver of the car sustained minor injuries and the driver of the lorry was unhurt.</p>
<p>There was a road block for almost 10 hours to allow initial investigation to be carried out.</p>
<p>Wiltshire Police has requested witnesses to come forward.</p>

<p>The man is the driver of the other vehicle involved in the collision and had fled the scene following the crash.</p>
<p>The accident took place near Esher Common at around 23:30 BST.</p>
<p>According to the police, two women sustained injuries in the crash.</p>
<p>The crash involved a Toyota Previa and a Peugeot 308.  It is believed that the man was driving a Toyota Previa.</p>
<p>He was seen fleeing the accident scene on foot.</p>
<p>Police are trying to trace the man.  Paramedics attended the accident scene and after receiving immediate medical attention, the women were discharged.</p>
<p>Police has requested any person who witnessed the accident to contact them.</p>

<p>A textiles company in West Yorkshire has been fined over safety failings after a teenaged worker sustained injuries as a result of getting trapped under a 1.5 tonne pallet.</p>
<p>On 26 April 2013, Halifax Magistrates Court was told that the teenaged worker suffered a broken leg as a result of the accident which took place on 16 March 2011.  The worker was 18 years old at the time of the accident.</p>
<p>The worker who has chosen to remain unnamed was admitted in hospital for three days.  Following his recovery, he has found job in another company.</p>
<p>An investigation carried out by the HSE revealed that there were serious safety failings which led to the accident.</p>
<p>The court heard that the day the incident occurred, the teenaged warehouse worker was inside a container and unloading pallets.  It was then a double stack of pallet fell on him and his legs became trapped underneath the load.</p>
<p>HSE found that the company had a dangerous system of work in place for unloading the pallets.  They also did not provide workers with proper equipment to enable them to do their tasks safely.</p>
<p>The pallets were mostly dragged by workers across the container floor using straps.  When they got the pallets close to the doors, forklift trucks would lift them.  The investigation also found that the forklift truck being used for lifting the pallets did not have a valid lifting certificate.  In addition, it was carrying loads heavier than its capacity when the incident took place.</p>
<p>The company had already been informed by another worker that the loads being lifted were too heavy even before the incident had occurred.  The company however ignored the warning.</p>
<p>THS Industrial Textiles were ordered to pay a fine of £4,994 for breaching Health and Safety at Work Act.</p>

<p>The Health and Safety Executive has prosecuted a national lift and elevator company for serious safety failings which were uncovered following the death of a lift engineer.</p>
<p>On 5 October 2010, the engineer who was working at Pentonville Prison died whilst attempting to fix a fault on the lift leading to the chapel.  He had failed to isolate the lift from the power supply.  He died of electric shock and was found dead on the same day by a prison employee.</p>
<p>On 15 April 2013, ThyssenKrupp Elevator UK Ltd was ordered to pay a fine of £100,000 with an additional £25,748 in costs for breaching safety legislation which was identified by the HSE during an investigation.</p>
<p>Southwark Crown Court heard that the 53 year old engineer died as a result of coming into contact with a live conductor and metal components of the lift simultaneously.  This created a passage to earth and gave him an electric shock which turned out to be fatal.</p>
<p>The HSE investigators, during their investigation found that the power supply to the lift had not been cut off and the multimeter used by the engineer had exposed tests probes which had the potential to lead to flashovers.</p>
<p>TKE Ltd, the maintenance contractor for prison service lifts in the United Kingdom had failed to provide adequate protection to employees who had to work on live electric systems.  The HSE found that the company failed to adopt a safe system of work and also failed to provide sufficient instructions and information to the employees regarding work on electrical systems.  In addition, the employees were left to work on their own without adequate supervision.</p>
<p>TKE had also failed to ensure that Mr Loake’s tools and equipment were fit for use. There was an increased risk of shock from the multimeter he used because it was not maintained in good condition.  Moreover, it lacked insulation which exposed excessive amounts of metal.</p>
<p>Southwark Crown Court was also told that the company was fined £233,000 in 2010 after a member of the public had died in an incident in March 2003.  The incident had taken place at the Broadgate Health Club and involved a lift which the company was responsible for maintaining.</p>
